What is the Levine PhenoAge formula?+

PhenoAge combines chronological age with nine routine blood markers, each carrying its own weight, into a Gompertz mortality model. The model produces a mortality risk score, which is then converted back into the age at which that risk is average: your phenotypic age. It comes from Levine et al. (2018), the paper that also underpins the DNAm PhenoAge epigenetic clock.

Which nine blood markers does PhenoAge use?+

Albumin, creatinine, glucose, hs-CRP, lymphocyte percentage, mean cell volume (MCV), red cell distribution width (RDW), alkaline phosphatase and white blood cell count, plus your chronological age. Every one of them appears on a standard complete blood count and comprehensive metabolic panel, with hs-CRP the only marker some labs leave off by default.

Do I need a special test, or will my normal blood work do?+

Normal blood work does. A CBC with differential plus a CMP covers eight of the nine; hs-CRP is a common add-on that most labs run for a few dollars. You do not need an epigenetic or methylation test to use this calculator.

What does it mean if my phenotypic age is lower than my real age?+

It means the nine markers together look like those of an average person younger than you: a favourable pattern, and in the source cohort a lower risk of death over the following years. It is a population-level statistical statement, not a prediction about you specifically, and a single panel is a snapshot: the trend across several tests is far more informative than one result.

How is this different from a lifestyle biological age calculator?+

A lifestyle calculator infers your ageing from habits: sleep, exercise, diet, smoking. PhenoAge measures the downstream biology those habits produce, using markers drawn from your blood. The lifestyle version is free and instant but rough; PhenoAge needs a blood draw and is correspondingly closer to what is actually happening in your body.

Is PhenoAge the same as an epigenetic clock like Horvath or GrimAge?+

No, though they are related. Epigenetic clocks read DNA methylation patterns and cost a few hundred dollars. PhenoAge is calculated from ordinary blood chemistry. The confusion arises because DNAm PhenoAge (a methylation clock) was trained to predict this phenotypic age score, so the two share a name and a lineage but not a method.

Can I enter my results in SI units?+

Yes. Each marker accepts either conventional US units or SI, and the conversion happens before the formula runs, so a European or Indian lab report works without you converting anything by hand.

What PhenoAge measures, and why it beats a birthday.

Phenotypic Age (PhenoAge), developed by Morgan Levine and colleagues in 2018, converts nine routine blood markers plus your chronological age into a single number: the age at which your measured physiology is "average" in the population. Someone who is 50 by the calendar but has the bloodwork of a typical 42-year-old has a PhenoAge of 42, and statistically the mortality and disease risk that goes with it.

Its appeal is that it is built entirely from cheap, standard labs (albumin, creatinine, glucose, hs-CRP, lymphocyte %, MCV, RDW, alkaline phosphatase, and white blood cell count) yet predicts mortality, healthspan, and disease onset better than chronological age, rivaling far more expensive epigenetic clocks. That makes it the most practical aging clock most people can actually run.

Why the number is easy to get wrong.

PhenoAge is not a simple average. It feeds the markers into a weighted linear combination, then through a Gompertz mortality function, then back-transforms the result into an age, and every marker must be in the exact units the formula expects (albumin in g/L, creatinine in µmol/L, glucose in mmol/L, the natural log of CRP in mg/dL, and so on). Get a single unit conversion wrong and the answer can be off by a decade, which is exactly why generic calculators and chatbots routinely botch it. This tool does the conversions for you from US or SI inputs.

Treat the result as a trend, not a verdict. Glucose and hs-CRP usually move it the most, and both respond to losing visceral fat, exercise, sleep, and better insulin sensitivity. Re-run the same panel every few months and watch the direction of travel.
